you may please fill out the forms attached if having accomplished all the requirements for PhD Candidate (all the elective and required courses completed, except for PhD dissertations, and passed the required QEs) and give them (physical documents only) to me (with your QE results and transcript) by 17:00  Feb. 21st.  ur application will be reviewed by the academic committee.  
please note that the first document must be signed by your advisor, no advisor signature no application acceptance.
